Introduction

Congratulations on your upcoming move to Kauai and the Pacific Missile Range Facility (PMRF). Kauai is a beautiful island with so much to offer! However, before you can enjoy that beauty, you have to get here! The 2010 PMRF Family Support Group made up of military spouses living on the island of Kauai worked together to create this moving guide as a reference for your upcoming move. It is information that we spouses have found helpful both prior to and during our stay here on Kauai.

Although Hawaii is a part of the United States, it is considered an overseas or OCONUS (outside continental United States) location. This means that there are MANY more steps involved in planning your move. Trust us when we say you will want to get started planning right away!

The information contained in this guide is intended as a starting point only! Please visit the websites listed as well as your local Fleet and Family Support Center and Personal Property Office for more detailed information and forms. If you feel something in this guide needs to be updated, please let me know by posting a comment.

Overseas Screening (OSS)

What is it?
The Overseas screening (OSS) looks at medical, dental, educational and potential duty limiting conditions or requirements of both service and family members. This is done because of the sometimes limited amount of healthcare found in remote overseas areas. Sometimes an overseas tour will not be in your family’s best interest. This will be determined during the screening. *Also see p. 14 of this guide.

When and How Is It Completed?

The Overseas Screening needs to be done within 30 days of receiving your official orders. This means you need to contact your local Military Treatment Facility (MTF) immediately after receipt of your orders. Please be aware that in large military areas, Overseas Screening appointments may take longer than 30 days to obtain. In extenuating circumstances such as this, an extension of the OSS can be filed by the CO of the new command. In addition, you will want to be sure you and your family are up to date on all dental cleanings and exams as most dentists won’t approve you otherwise.

Exceptional Family Member (EFM) Screening

Ensure eligible family members with special needs, whether it be medical, educational or both are enrolled in The Exceptional Family Member Program. Members of this program will have to go through a separate overseas screening process. Please contact your local Military Treatment Facility (MTF) for more information. *Also see the Medical Section of this guide.

Enhanced Post-Deployment Health Assessment

If the active duty member is arriving in Kauai on the heels of a deployment, he or she may also need to undergo an Enhanced Post-Deployment Health Assessment (PDHA) Process (DD Form 2796). Dependent Entry Approval

Once the active duty member and the dependents have completed and passed the overseas screening, the CO of the new command (PMRF) will need to send a Report of Suitability for Overseas Assignment, (NAVPERS 1300/16) to the active duty member’s current command. This can be completed only when the member and dependents, if any, are considered suitable in all respects for duty overseas.

Requesting a Dependent Entry Approval
To request a Dependent Entry Approval from PMRF, please contact your current ADMIN office and ask that a request for a Dependent Entry Approval be sent through message traffic to the ADMIN Office of the Pacific Missile Range Facility, Barking Sands, Kauai. Do not be surprised if your current ADMIN does not know what a Dependent Entry Approval request is, as most of the lucky PMRF residents have found! In many of our cases, the active duty members had to walk their ADMIN offices through the process.

Once the PMRF ADMIN receives this message, they will send out the Dependent Entry Approval via message traffic to the active duty member’s current squadron. You will then need to go to your current ADMIN office to get a copy of this document. Make several copies and have them on hand as you will be asked for it MANY different times throughout the moving process. You will need this document prior to setting up a Household Goods move, shipping your vehicle, or booking your airline tickets through the military.

Everyone living on base shares the same PO Box number for regular USPS mail:

Mail is divided into separate family boxes upon arrival at PMRF.

For UPS, Fedex, or DHL please use your physical residence address.

**Many online websites, UPS, FedEx, and DHL may not recognize our base residence addresses as valid. Often times a phone call to the company will resolve the issue. It is also wise to include C/O PMRF in one of the address lines. This will alert the companies to the fact that the address is on our military base.

Guests/Visitors: If you would like to sponsor someone on base including service repair or installation personnel, family, friends, or any other type of guest, for ONE DAY, an email must be sent to Pass and ID at pass&id@pmrf.com. The email needs to include the following information: (a) Driver or Company Name, (b) Name of Sponsor, (c) Sponsor’s Address, and (d) Expected Date and Time of Arrival.

When the guest arrives at Pass and ID they will need to have the following to gain access to base: (a) ID for themselves and anyone else in the vehicle, (b) current registration in the DRIVERS name, (c) current safety check, and (d) insurance card.

Please be aware that a phone call will not get your visitors on base. You must notify pass and ID about visitors via email.

For overnight stays between 1-30 days, the sponsor must obtain an ID card for the guests from Pass and ID. Sponsor must complete the guest form from Pass and ID and obtain the signature of the Security Officer.

For stays of 30 days or more, the CO and Security Officer’s signatures are required.

Upon completion of the form, sponsors must return the form to Pass and ID to obtain an ID card and pass for their guest.

Cable/Phone/Internet:

Time Warner Cable : be sure to call and set up an appointment BEFORE arriving in Kauai as new customer appointments can take as long as 6 weeks to book.

Direct TV : Dishes are authorized on base, but satellite access to signal at certain housing locations may be limited or non-existent. Dish may not be mounted on roof. Cables must be fed through the window. Direct TV is not allowed to drill through our walls. You may have cables fed along your fence or through your grass. If you chose to bury cables, it is wise to enclose them PVC piping first to extend the cable’s life. (800) 943-2046

Utilities
Water, gas, and trash service are included in your rent. Electricity is included as long as you stay within the average based on your housing unit. Residents are responsible for paying electricity costs above the average housing consumption. There is a $100 monthly rebate available to families who keep their electricity below the average use.

Renters Insurance
There is $35,000 of Basic Personal Property Insurance Coverage ($250 deductible) included in your rent.

Painting
You can fill out a request at the PMRF Housing Office to make minor modifications to your home such as painting, adding shelves to garages, etc., but modifications must be removed or returned to normal conditions when you vacate.

Pets
Cats, dogs, birds, fish, and guinea pigs are permitted in our neighborhood. Reptiles, rodents (except guinea pigs), ferrets, rabbits, pigs, and spiders are NOT permitted. In addition, Pit Bulls, Rottweilers, and several types of Terriers are also not permitted. Garages/Parking
Your housing unit comes with a detached one car garage and one extra parking space.

Yard Responsibilities

Your front yard will be maintained by Forest City, but you are responsible for the maintenance of your backyard. There is a lawn equipment loaner program, although many of us have found it is more efficient to purchase your own equipment. Despite the fact that we have an excellent Forest City staff, many times the loaner equipment is broken or missing due to user negligence. Automatic sprinklers come on during the night. All yards are fenced.

Washer Dryer Hookups
Locations vary depending on which floor plan you have, but the hookups for the majority of houses are located in a large closet area of the lanai (see below). Washers and dryers are provided and included in your BAH here, but you are welcome to bring your own if you wish.

What is a Lanai?
Our houses on PMRF include a lanai, which is a fully enclosed patio in the back of the house. Your lanai windows and door may be screened OR enclosed in Plexiglas. If your lanai does not have Plexiglas, you are permitted to install it. There is no air conditioning in the lanai, however, windows and door from the living room area open onto the lanai and it can be kept relatively cool when Plexiglas is installed.

Pet Travel

Hawaii Quarantine Laws
If you are bringing a pet to Hawaii, particularly a dog or a cat, it is VERY important that you review Hawaii’s pet quarantine laws ASAP. There are six major requirements that must be met in order for your pet to avoid a lengthy 120 day quarantine on-island.

1. Two Rabies Vaccinations
2. Microchip Placement
3. OIE-FAVN Rabies Blood Test
4. 120 Day Waiting Period (approx. 4 months)
5. Gathering Proper Documents
6. Submission of Documents

If any of these specific requirements are not met for your pet PRIOR to arrival in Hawaii, your pet will be quarantined at your expense for up to 120 days. If you take the proper steps to ensure your pet meets Hawaii’s pet import requirements, you will qualify for the 5-day-or-less Quarantine Program.

Pets on the Airlines

Each airline has different prices, requirements, and regulations regarding pet travel. In some cases, your dog or cat may be able to ride in the cabin, in other cases, they may be put in the cargo hold. It will be up to you to research which airline is best for your pet. Most airlines charge by crate size and total weight including the crate. DO NOT try to cram your pet into a crate that is too small! Pay particular attention to crate size, as most airlines require that your animal must be able to stand fully and turn around in the crate. In addition, if you are obtaining airline tickets through the military, be sure that they book you on the correct AIRLINE and flight number. It will be your responsibility to call beforehand and be sure there is room for you and your pet on the flight. Also, if your pet is being quarantined in Honolulu, be sure that your flight includes a long enough layover in Honolulu and does not go straight through to Lihue.

Picking Up Your Pet
If you qualify for the 5-Day-or-Less Program, you have the option of picking your pet up in Lihue, Kauai for an extra charge. However, if you choose this option, your paperwork has to be sent in 30 days prior to your arrival date. If you do not qualify for the 5-Day-or-Less Program, your pet will be quarantined in Honolulu and you will need to sign him/her in. The airline will bring the animal to a small quarantine office on the outskirts of the airport where you will be able to check on the animal briefly. After signing them in, you will continue on to Lihue and catch a flight back to Honolulu when the quarantine is over. The quarantine facility is about a mile away from the airport, so this will require renting a car or finding a taxi big enough to accommodate your animal’s crate. Airport laws do not allow you to remove your pet from the crate at any time. You will have to drive your pet from the Quarantine facility back to Honolulu Airport and then catch your flight back. Be sure to give yourself ample time to do so.

Kauai Lodging

PMRF Beach Cottages
If you need a place to stay while looking for a house, or have family or friends coming to visit, the PMRF Beach Cottages are the best lodging deal on the island! You can sponsor your friends or family into a cottage and reservations can be made up to 6 months in advance for Active Duty Military. Other Lodging
Most hotels on the island offer either military or Kama Aina (HI resident) discounts. Be sure to ask before booking to save some money!

Car Registration/ Hawaii Driver’s License

Registration
Don’t be fooled by private online vehicle registration companies! It is far easier, faster, and CHEAPER to wait to register your vehicle in person.

Upon your vehicle’s arrival, you have 30 days to obtain a safety check and register your car. Safety checks can be obtained for about $15 from Mana Motors on PMRF (behind the gas station) or from any authorized auto shop. Safety checks are also required before a PMRF auto decal can be issued.

Military members can register their vehicles in Hawaii for $5.50 and renew their vehicle registration for 50 CENTS by obtaining a Military non-resident form from ADMIN. This form must be signed by the military member! Active duty member’s name must also be on the vehicle’s registration to qualify for the military discount.

Driver’s License/ Hawaii ID/ Kama Aina Discounts
You will have to take a written test in order to obtain a Hawaii driver’s license. A driver’s license or Hawaii ID is like a discount card for many stores and attractions in the area. Many tourist-y places offer Kama Aina (resident) discounts, some of them up to 50% off. Your Hawaii ID or Driver’s license is your proof of residency for Kama Aina rates.

Shipping Your Vehicle

You will need to prepare your vehicle to be shipped according to the shipper’s guidelines which can be obtained from your local Personal Property Office. They will give you a list of shipping companies the military works with and you will choose the one nearest you. In some cases, it’s in the same town, in other cases, it may be a few hours away. You MUST have an appointment to drop off your vehicle as well as the proper paperwork. Some companies require that the paper work be faxed in several weeks ahead of time, so don’t delay in contacting the shipping company for all the specifics!

Shipping times for vehicles vary depending on the point of origin. MOST of our residents have found that their vehicles arrived 3-6 weeks after the initial drop off.

As of now, a typical inter-island flight will cost you about $150-$200 round trip. There are deals and specials that run throughout various seasons, just keep an eye out! Some of the most common inter-island flights run on:

**If you are anticipating making major purchases upon arriving, we advise against it! Selection in Kauai is quite limited, and many online stores either refuse to ship to Hawaii (ie: Costco, Walmart) or charge an arm or a leg for shipping. Plan ahead as much as possible and get what you need BEFORE arriving in Kauai!

**Many PMRF residents choose to purchase an extra refrigerator or freezer to store their bulk food in and to cut down the number of trips to Lihue. Buying in bulk is almost a necessity here.

Located on the magical north shore of the Garden Island of Kauai, Kauai Kunana Dairy is the only dairy on the island. With a milking herd of only twenty does, they are a true micro-dairy, specializing in artisan goat cheese and goat milk products. The family farm also produces USDA/HOFA certified organic fruits and vegetables in addition to delightful pestos, dressings, and baked goods.

Youth Center / Child Development Center (CDC)

It is very important to contact the PMRF CDC as soon as you get your orders. It is a small facility and they need advance notice to prepare for families when they arrive. It is best to fill out a request for care form and fax it to the CDC at 808-335-4568.

The CDC currently offers full time or part time slots. Part time (pre-K only) slots include TU/TH 7:30am-12pm or MWF 8am-11:30 am. Hourly care is also available for $3.00 per hour, with a minimum charge of $6.00.

The Youth Center offers free after school programs for kids in K-12 grade as well as Furlough Friday Programs and Seasonal Day Camps for a fee. The Free Furlough Friday program is for single or dual active duty or active duty with full time working or full time student spouse only. This is because of a grant that Hawaii received to cover the costs. It is unknown if this grant will be available for the 2010/20111 school year.

For your convenience, you will want to be sure your child is up to date on vaccinations and has a CURRENT TB test BEFORE you arrive! Your child will NOT be allowed to attend the CDC without these requirements met.

Once a year, a military vet from Oahu comes to PMRF to provide care for our animals. You will be notified by email when this occurs. If you should need a vet any other time during your stay on Kauai, we recommend the following:

Medical Information

As mentioned before, it is critical that all family members pass the Overseas Screening (OSS). Please be advised that medical care on Kauai is VERY basic! If you have any type of medical specialist- based needs, plan ahead! You will want to become VERY familiar with what Tricare covers and what they do not! Tricare will cover your medical travel expenses should you need to fly to another island for medical care (and many people do!) Common Hawaiian Medical Issues
The following medical issues may be encountered during your time here. These are the most common issues we PMRF spouses have had to deal with. Descriptions are retrieved from www.webmd.com.

Lice/Ukus:
What causes a lice infestation?
Lice spread easily from one person to another through close contact or through shared clothing or personal items (such as hats or hairbrushes). A louse cannot jump or fly.
What are the symptoms?
The most common symptom of lice is itching There are different symptoms, depending on which type of lice you have.
• Head lice may not cause any symptoms at first. Itching on the scalp may start weeks or even months after lice have started to spread. Scratching can make the skin raw. The raw skin may ooze clear fluid or crust over, and it may get infected.
• Pubic lice cause severe itching. Their bites may cause small marks that look like bruises on the torso, thighs, or upper arms. If pubic lice get on the eyelashes, the edges of the eyelids may be crusted. You may see lice and their eggs at the base of the eyelashes.
• Body lice cause very bad itching, especially at night. Itchy sores appear in the armpits and on the waist, torso, and other areas where the seams of clothes press against the skin. The lice and eggs may be found in the seams of the person's clothing but are generally not seen on the skin.
Frequent scratching can cause a skin infection. In the most severe cases of head lice, hair may fall out, and the skin may get darker in the areas infested with lice.
How is a lice infestation diagnosed?
A doctor can usually tell if you have lice by looking closely for live lice or eggs in your hair. He or she may look at the lice or eggs under a microscope to be sure.
Your doctor can also find pubic lice and body lice by looking closely at your body or your clothing.
How is it treated?
The most common treatment is an over-the-counter or prescription cream, lotion, or shampoo. You put it on the skin or scalp to kill the lice and eggs. In some cases, you may need treatment a second time to make sure that all the eggs are dead. If two or more treatments don't work, your doctor may prescribe a pill called ivermectin.


Molluscum:
Molluscum contagiosum is a viral skin infection that causes either single or multiple raised, pearl-like bumps (papules) on the skin.
Molluscum contagiosum is a chronic infection and lesions may persist from a few months to a few years. However, most cases resolve in six to nine months.
Causes of Molluscum Contagiosum
Molluscum contagiosum is caused by a virus (the molluscum contagiosum virus) that is part of the pox virus family. The virus is contagious through direct contact and is more common in children. However, the virus also can be spread by sexual contact and can occur in people with compromised immune systems. Molluscum contagiosum can spread on a single individual through scratching and rubbing.
What Are the Symptoms of Molluscum Contagiosum?
Common locations for the papules are on the face, trunk, and limbs of children and on the genitals, abdomens, and inner thighs of adults. The condition usually results in papules that:
• Are generally painless, but can itch
• Are small (2 to 5 millimeter diameter)
• Have a dimple in the center
• Are initially firm, dome-shaped, and flesh-colored
• Become softer with time
• May turn red and drain over time
• Have a central core of white, waxy material
Molluscum contagiosum usually disappears spontaneously over a period of months to years in people who have normal immune systems. In people who have AIDS or other conditions that affect the immune system, the lesions associated with molluscum contagiosum can be extensive.
Diagnosing Molluscum Contagiosum
Diagnosis of molluscum contagiosum is based on the distinctive appearance of the lesion. If the diagnosis is in question, a doctor can confirm the diagnosis with a skin biopsy -- the removal of a portion of skin for diagnostic examination). If there is any concern about related health problems, a doctor can check for underlying disorders.
Treatment for Molluscum Contagiosum
The condition is usually self-limited, so treatment is not always necessary. However, individual lesions may be removed by scraping or freezing. Topical medications, such as those used to remove warts, may also be helpful in lesion removal.
Note: The surgical removal of individual lesions may result in scarring.
Prevention of Molluscum Contagiosum
To prevent molluscum contagiosum, follow these tips:
• Avoid direct contact with anyone who has skin lesions.
• Treat underlying eczema in children.
*As a side note, be sure to thoroughly scrub your kids down after playing in inflatable bounce houses and waterslides, as these seem to be breeding grounds for molluscum here in Kauai.

Impetigo:
What Is Impetigo?
Impetigo is a highly contagious bacterial skin infection. It can appear anywhere on the body but usually attacks exposed areas. Children tend to get it on the face, especially around the nose and mouth, and sometimes on the arms or legs. The infected areas appear in plaques ranging from dime to quarter size, starting as tiny blisters that break and expose moist, red skin. After a few days the infected area is covered with a grainy, golden crust that gradually spreads at the edges.
In extreme cases, the infection invades a deeper layer of skin and develops into ecthyma, a deeper form of the disease. Ecthyma forms small, pus-filled bumps with a crust much darker and thicker than that of ordinary impetigo. Ecthyma can be very itchy, and scratching the irritated area spreads the infection quickly. Left untreated, the sores may cause permanent scars and pigment changes.
The gravest potential complication of impetigo is post-streptococcal glomerulonephritis, a severe kidney disease that occurs following a strep infection in less than 1% of cases, mainly in children. The most common cause of impetigo is Staphylococcus aureus. However, another bacteria source is group A streptococcus. These bacteria lurk everywhere. It is easier for a child with an open wound or fresh scratch to contract impetigo. Other skin-related problems, such as eczema, body lice, insect bites, fungal infections, and various forms of dermatitis can make a person susceptible to impetigo.
Most people get this highly infectious disease through physical contact with someone who has it, or from sharing the same clothes, bedding, towels, or other objects. The very nature of childhood, which includes lots of physical contact and large-group activities, makes children the primary victims and carriers of impetigo.

Jellyfish Stings:

Can be present at certain times of year on PMRF beaches. If you are stung, seek medical attention immediately. Meat tenderizer can be applied to aid with the pain.


Allergies: Stock up or refill your allergy prescriptions! Kauai flora and “vog” (volcanic ash and fog) make for some serious Kauai allergy seasons!
